Lost Your COVID-19 Vaccination Card, Now What?

According to the US Centers for Disease Control and Prevention (CDC), an easy way to confirm that someone has been vaccinated against COVID-19 in the USA right now is a physical paper card.

This vaccination card includes medical information such as the type of COVID-19 vaccine you received, the date you received it, and where you were vaccinated, stated the CDC on March 25, 2021.

If you do not receive a COVID-19 vaccination card at your appointment, contact the vaccination provider or your state health department to determine how you can get a card.

And ask your vaccination provider about getting started with V-safe, a free, smartphone-based tool that uses text messaging and web surveys to provide personalized health check-ins after you receive a COVID-19 vaccination.
